Explained: The Form Every Italian Must Carry During the National Lockdown

Italy has announced that its lockdown measures will be tightened in an effort to keep more people home and slow the spread of the novel coronavirus.

The quarantine measures stipulate that everyone in the entire country must stay home as much as possible, exiting only for necessities such as food, medicine, and health visit.

Anyone who leaves their home must carry a self-declaration form, which can be found here. The form can be printed or hand-copied and must be presented to any authority on demand.

The official form can be found here, with an explanation of what Italians are being asked to legally declare included below.

Lying on the form is an offense that is punishable by fines or jail time.

The English translation of the form is as follows.

OFFICIALLY DECLARE THAT I AM:

➢ aware of the contagion containment measures referred to in the combined provision of art. 1 of the Decree of the President 8 March 2020 and
art. 1, paragraph 1, of the Decree of the President of 9 March 2020 concerning the movement of persons throughout the national territory;
➢ not subject to the quarantine measures and not to have tested positive
for the COVID-19 virus referred to in article 1, paragraph 1, letter c), of the President’s Decree of 8 March 2020;
➢ aware of the penalties provided for by the combined provisions of art. 3
paragraph 4, of Legislative Decree February 23, 2020, n. 6 and art. 4, paragraph 2, of the Decree of the President of 8 March 2020 in the event of noncompliance with the aforementioned containment measures (Article 650 of the Italian Criminal Code unless the actions constitute an even more serious crime);

that my movement is determined by:

  • proven working needs;
  • situations of necessity;
  • health reasons;
  • return to your home or residence.

This form replaces an earlier version of the quarantine form that was first released on March 9th by adding the clause which forces people to declare that they have not tested positive. Those who have tested positive are ordered to stay home for a full quarantine and cannot leave their house for any reason.
